Assumed Risk Statement
CCA strives to keep the safety and health of students and athletes as a priority. With any athletic activity or organized sport, there is always a risk of injury, minor or extreme, due to physical exertion, bodily contact, or equipment malfunction. Students and parents must assume this risk that bodily injury can happen during a practice, game, or travel.
Participation inclues possible exposure to and from infectious diseases. While particular rules and personal discipline may reduce this risk, the risk of serious illness does exist.
Due to the possibility of injury, each parent needs to have medical insurance on their student.
This is to certify that I have read the assumed risk statement and certify that I am the parent/guardian for this participant and furthermore agree to accept these risks. I give permission for my child to participate in this event in accordance with the details written in this registration form.
