Elementary B.Ball permission slip

The permission slip needs to be filled out and signed by a parent or guardian if you would like your child(ren) to participate in the elementary basketball program. The students in grades 3-6 will be given the opportunity to sign up at school February 27th. This is a special event they are very excited about.

The permission slip must be submitted by March 6th.
